We are seeking a Full Time Remote Patient Assessment Coordinator to join reputed company! • Remote position but must reputed company in reputed company Dakota or Colorado** Hospital Details Vibra Hospital of Central Dakotas, located in Mandan, ND is a 41 bed Critical Care Hospital, providing long-term acute care services to patients recovering from serious illnesses or injuries. Often our patients require additional critical care services for medically reputed company conditions such as trauma, infectious diseases, wound healing, cardiovascular disease, stroke, amputation and ventilator weaning. As a smaller, specialized hospital, we offer an opportunity to work in a fast-paced and friendly work environment where you really get to know your patients and their families. Responsibilities Conducts patient assessments to identify patients for potential admission into reputed company Hospitals. Maintains reputed company relationships with reputed company referral sources. Coordinates reputed company of clinical and benefit information from referral sources back to hospital admissions team, administration and reputed company party payers. Essential responsibilities include: performing clinical assessments, expanding the referral reputed company and improving customer and community relationships.
