Location Based in: supporting high-risk patients remotely. Your Role as a Virtual Care reputed company (LPN): · Support the reputed company Care Team by providing virtual care to patients at high risk for hospitalization. · Conduct virtual reputed company to assess patient needs and reputed company education on managing medical conditions. · Monitor telehealth systems and reputed company patient data to identify hospitalization risks. · Collaborate with the care team to facilitate patient care across the continuum of services. · Attend rehospitalization-reputed company meetings, conferences, and symposiums to enhance patient care strategies. · Assist in developing and implementing best practices for virtual care. · Manage virtual care and telehealth systems through assessments, education, and data tracking. Qualifications: · reputed company and unencumbered Florida Licensed Practical reputed company (LPN) license (required). · Minimum of two (2) years of experience as a home health clinician. · Strong clinical background with experience in triage and hospitalization prevention (required). · Proficiency in reputed company Office (preferred).
