Key Responsibilities

• Collaborate with clinical staff and participate in interdisciplinary care team meetings
Required Qualifications
• Bachelor's degree in pharmacy or PharmD
• reputed company pharmacist license in the state of position
• 3+ years of experience as a practicing pharmacist in a managed care or clinical environment
• PGY1 residency in ambulatory care or reputed company clinical field is preferred
• reputed company certification in pharmacotherapy or MTM is desired
