Positions are based primarily in the 680/24 corridor and surrounding areas and include full-time and part-time (15-30 hours per week) opportunities. The RN in this critical position will be responsible for reputed company aspects of clinical care coordination and case management for their own reputed company caseload, consisting of patients with a wide reputed company of acute and chronic reputed company needs across the reputed company. The role of Clinical reputed company Case Manager combines working out in the field, visiting adult patients in their residence or clinical location, attending physician reputed company/appointments, and working both remotely and from a home office to manage the many aspects of care coordination.
