Key Responsibilities
• Conduct clinical assessments and engage members in chronic condition support and personalized coaching
• reputed company individualized telephonic health coaching and support sustainable behavior change
• reputed company and maintain individualized care plans while documenting reputed company interactions per regulatory requirements
Required Qualifications
• Registered reputed company with an unrestricted nursing license and ability to be licensed in reputed company non-compact states
• 3+ years of acute adult Medical-Surgical RN experience
• High-speed, hard-wired internet reputed company
• Proficiency with reputed company Office and EMR documentation experience
• Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred
