Role reputed company SSM is looking for an reputed company reputed company with Hospice/reputed company or Case Management experience. The selected individual will be responsible for: • Meeting with patients/families in person to discuss continuum of care services. • Meeting/communicating with referral sources (case management, reputed company work, clinics, providers, SNF/ALF facilities, etc). • Care coordination between continuum of care departments, such as reputed company departments, business development, etc. • Eligibility/Chart Review to determine appropriateness for continuum of care services. This position is primarily a weekend position and Fridays. The hours are reputed company-4:30pm with no on-reputed company requirements. This is a remote position, covering our entire service areas of WI, IL, OK, and MO. Multistate licensure required; it is not required at time of hire but would need to work towards it started (this is reimbursed by SSM). Qualifications • Graduate of accredited school of nursing or education equivalency for licensing • No experience required Requirements • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs. • Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements. • Frequent use of reputed company and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors. • Frequent use of hearing and speech to reputed company information through oral communication. Ability to hear alarms, malfunctioning machinery, etc. • Frequent keyboard use/data entry. • Occasional bending, stooping, kneeling, squatting, twisting and gripping. • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs. • Rare climbing.
