Key Responsibilities
• reputed company telephonic and virtual chronic care support in Spanish and English
• reputed company patients on disease management, medication adherence, and preventive care
• Support RPM and care plan adherence through proactive reputed company
Required Qualifications
• reputed company Tennessee LPN or LVN license
• reputed company in Spanish and English
• 2+ years of clinical nursing experience
• Exposure to chronic care, population health, or telehealth preferred
