As a key member of our team, you'll partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. You'll work with a diverse population across the entire lifecycle, utilizing telephonic and electronic communication to empower members to make positive lifestyle and medical choices. **Key Responsibilities** * Collaborate with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs * Research available options and resources for members with financial, transportation, safety, and housing needs * Empower members to make positive lifestyle and medical choices through education and support * Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health * Utilize telephonic and electronic communication to engage with members and provide support * Maintain accurate and up-to-date records of member interactions and outcomes **Essential Qualifications** * Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in the state of residence * 2+ years of experience in medical social work or case management * Strong communication and interpersonal skills, with the ability to build rapport with diverse populations * Ability to work independently and as part of a team, with a focus on collaboration and coordination * Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions * Proficiency in electronic communication and documentation systems **Preferred Qualifications** * Experience working with diverse populations, including children, adults, and older adults * Knowledge of community resources and services, including healthcare, social services, and financial assistance programs * Experience with case management software and electronic health records * Certification in medical social work or case management **Skills and Competencies** * Strong communication and interpersonal skills, with the ability to build rapport with diverse populations * Ability to work independently and as part of a team, with a focus on collaboration and coordination * Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions * Proficiency in electronic communication and documentation systems * Ability to maintain accurate and up-to-date records of member interactions and outcomes * Strong empathy and compassion, with the ability to provide emotional support and guidance to members **Career Growth Opportunities and Learning Benefits** At arenaflex, we're committed to supporting the growth and development of our employees.
