Provide compassionate and efficient care coordination services to Aetna members. Work closely with healthcare providers to ensure proper care and support is provided to members. Collaborate with members to develop personalized care plans that address their specific needs and goals. Utilize knowledge of healthcare systems and resources to assist members in navigating their healthcare journey. Conduct ongoing assessments of members' needs and make appropriate referrals to other healthcare professionals. Monitor and track members' progress and outcomes to ensure the effectiveness of care plans. Educate members on their healthcare benefits and assist them in accessing necessary services. Serve as a liaison between members, healthcare providers, and insurance companies to coordinate and facilitate care. Maintain accurate and detailed documentation of all interactions and interventions with members. Stay up-to-date on industry trends and regulations related to care management and ensure compliance with all applicable laws and regulations. Participate in regular team meetings and collaborate with other care management coordinators to ensure consistent and high-quality care delivery. Continuously evaluate and improve care management processes to enhance the overall member experience. Display empathy, patience, and professionalism when interacting with members and healthcare providers. Adhere to Aetna's mission, values, and ethics in all interactions and decisions.
