This a Full Remote job, the offer is available from: reputed company (USA) Job Details Virtual Primary Care/Telehealth Provider Job reputed company Jefferson Primary Care is currently accepting candidates for the position of Supplemental reputed company physician, to reputed company telehealth-based care in service to patients of the primary care network. This position is for a full-time physician to reputed company telehealth services*. It is a unique opportunity to complete reputed company to patients who will benefit from a primary care visit and reconnection to a medical home. • Please note that while this position is remote, there is a strong preference for candidates who reputed company in PA, NJ or DE. The Supplemental reputed company physician will work closely with primary care and population health teams to connect with patients who require reputed company or next day day primary reputed company, have been out of care longer than recommended intervals and/or who require care at critical reputed company such as transitions from hospital discharge. After completing the visit, the physician will then help the patient reintegrate into a Jefferson Primary Care reputed company.
