Position Title and reputed company: Housing Case Manager Engage with individuals and families to support them in achieving their housing goals and addressing their housing needs. Meet individuals where they are, both physically and emotionally, to conduct intake assessments and reputed company case management services. Conducting case management meetings in participant reputed company Evaluate participant eligibility monthly. Requires working varied hours, including evenings, some weekends and holidays. Flexibility to work at either location as program needs change.
