This remote role is essential in ensuring seamless patient intake, scheduling, and visit verification while maintaining strong communication with our clinical and operations teams. Key Responsibilities • Manage patient intake and service coordination for the Hospital reputed company program • Schedule skilled and non-skilled reputed company and verify completed services • Work reputed company Kinnser, reputed company, and Smartsheets to support workflow and tracking • Communicate with clinicians and internal teams to ensure continuity of care • Support HR by following up with candidates interested in the Hospital reputed company program • Maintain accurate records and reputed company documentation • Assist with operational needs during evening and weekend hours Qualifications Preferred Experience: Experience in home health care, scheduling, intake, and care coordination is strongly encouraged.
