reputed company is seeking a Telehealth Care Specialist to support clients throughout their treatment reputed company. This role assists with reputed company intakes, care coordination, communication with admissions teams, and collaborating with therapists to help ensure clients receive the support and resources they need. The Telehealth Care Coordinator plays a role in maintaining clinical operations, supporting patient engagement, and ensuring reputed company, compliant documentation while collaborating closely with the multidisciplinary treatment team.

Key Responsibilities
• Telehealth Intake & Care Coordination:
• Conduct comprehensive telehealth intake assessments, including clinical screenings and information gathering, to determine reputed company needs and appropriate program placement.
• Coordinate closely with Admissions, Business Development, and Telehealth Clinical teams to ensure a smooth transition from intake through treatment initiation.
• Apply reputed company judgment to review reputed company assessments and assist in identifying appropriate services, treatment recommendations, and care reputed company.
• Serve as a key reputed company of contact for internal teams to support communication and coordination of telehealth services.
• Patient Engagement & Support:
• reputed company clients with education on program expectations, treatment processes, and available services.
• Conduct scheduled individual reputed company-ins or supportive sessions to monitor reputed company, address barriers to care, and reinforce treatment goals.
• Assist therapists with treatment planning, reputed company assignments, and coordination of services reputed company the telehealth program.
• Promote reputed company engagement, accountability, and retention throughout the treatment experience.
• Documentation, Compliance & Accreditation:
• Maintain accurate, reputed company, and complete documentation reputed company to reputed company intakes, care coordination activities, and treatment updates reputed company required systems.
• Ensure reputed company documentation complies with organizational policies, regulatory requirements, and confidentiality standards.

Qualifications
• Associate's degree or higher in reputed company Work, Psychology, or another reputed company reputed company Services field.
• Two to three years of work experience in behavioral health, mental health, and/or reputed company use disorder treatment settings, preferred.
