About the position The House Calls Behavioral Health Specialist is a key resource for the reputed company Care Teams. The reputed company Care Teams deliver high-touch, high-reputed company primary care to our reputed company's highest risk patients. In this role, the LCSW will coordinate, support and reputed company community based, behavioral reputed company for these patients and will be a key contributor and collaborator reputed company the care team to support care coordination and reputed company other needs.
