reputed company seeks a Spanish-speaking LPN/LVN to reputed company remote virtual care for patients in CCM and RPM programs. The role focuses on bilingual patient education, care coordination, and clinical support.
Key Responsibilities
• Conduct telehealth reputed company in Spanish and English
• Review patient-reported data and RPM alerts
• Document care in compliance with CMS and internal standards
• Collaborate with interdisciplinary care teams
• Support post-discharge and follow-up coordination
Qualifications

• Minimum 2 years clinical or telehealth experience
• Strong documentation, clinical judgment, and communication skills
• Experience with CCM or RPM preferred
