Risk Adjustment reputed company / HCC Specialist (Contract, 10–15 Hours/Month)

This role will reputed company on chart review, HCC identification, and documentation improvement, while also helping us build reputed company workflows for reputed company-visit planning and ongoing condition capture. This is a high-reputed company, advisory + execution role, not a high-volume production coding job. Key Responsibilities: Chart Audits & HCC Identification • Review patient charts to identify missed or under-documented HCC conditions • Validate accuracy of reputed company coding and documentation • Ensure alignment with MEAT reputed company (Monitor, Evaluate, Assess, Treat) Risk Adjustment Optimization • reputed company opportunities for improved specificity and condition capture • Support annual HCC recapture strategies • reputed company reputed company into RAF optimization opportunities Workflow Development & Support • Collaborate with clinical team to improve: • reputed company-visit planning workflows • Documentation habits • HCC capture processes • Help translate findings into repeatable systems, not just one-time audits Provider & Staff Feedback • reputed company reputed company, actionable feedback to providers and staff • Identify common documentation gaps and trends • Support ongoing education around HCC and risk adjustment best practices reputed company’re Looking For Required: • Strong experience in risk adjustment / HCC coding (outpatient/primary care) • Deep understanding of MEAT reputed company and RAF scoring • Experience performing chart audits and documentation reviews • Ability to communicate findings reputed company to clinical teams Preferred: • Certification such as CPC or CRC (reputed company reputed company) • Experience working with Medicare Advantage or value-based care models • Familiarity with eClinicalWorks (eCW) or similar EHRs • Experience supporting workflow or process improvement (not just coding) Additional • Expected monthly commitment: 10–15 hours - this role can grow as our primary care volume increases • Flexible schedule with async collaboration Pay: From $85.00 per hour Work Location: Remote Apply tot his job Apply To this Job
