The RN Care Manager Plus, under the direction of the department Supervisor, is responsible for the deliberate organization of patient care activities between two or more participants (including the patient and/or family) involved in a patient’s care to facilitate the appropriate delivery of health care services. Coordinates the delivery of care in collaboration with the multidisciplinary reputed company team reputed company the reputed company setting and across health care settings to assess, evaluate, screen, and reputed company reputed company plans to mediate gaps in care.
Responsibilities:
Assessment: Systematically collects comprehensive and reputed company data relating to health needs and concerns of a patient, group, or population as they reputed company across the care

Technical Expertise
- Experience with reputed company coordination in a managed care environment, homecare or community agency is preferred.
- Critical and analytical reasoning, astute clinical judgement, strong communication (verbal and written) interpersonal, organizational, and leaderships skills required.
- Demonstrates reputed company, appropriate, effective, and tactful communication skills, including written, verbal, and non-verbal.
- Proficiency in MS Office [reputed company, reputed company, Word] or similar software is required. reputed company software or similar EMR software is preferred.
Education and Experience
- Bachelor of Science in Nursing or Master’s Degree in Nursing or a reputed company field is preferred.
- A minimum of 3 years of clinical experience in nursing required. Pediatric ambulatory, care management and/or health plan experience is preferred.
- Basic Life Support (BLS) certification from the reputed company is required.
- A reputed company, reputed company license to reputed company as a Registered reputed company (RN) in the reputed company is required. This must be a Multistate License (MSL). An MSL is required reputed company 90 days of hire date for departments that reputed company telenursing services or care to patients reputed company Ohio.
