We are currently interviewing for a <strong>PRN LVN Field Nurse</strong> to cover the Tyler, Chandler, Flint, Bullard, Lindale and surrounding areas.</div><div><br></div><div>We are looking for a Home Health LVN to provide skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse. This role supports the delivery of safe, high-quality patient care through routine nursing visits, treatment administration, patient education, and timely documentation. Previous Home Health Care experience is a must. Familiarity with electronic charting (KINNSER/AXXESS) is a plus.</div><div> <br><strong>Responsibilities:</strong> </div><ul> <li>Provide skilled nursing care to home health patients under the supervision of an RN</li> <li>Perform routine nursing visits including vital signs, medication administration, injections, and treatments</li> <li>Assist with wound care and dressing changes per physician orders</li> <li>Monitor patient condition and promptly report changes to the supervising RN</li> <li>Educate patients and caregivers on medications, treatments, and safety</li> <li>Accurately document all visits and care provided in the agency EMR</li> <li>Ensure compliance with Medicare, Texas regulations, and agency policies</li> <li>Coordinate visits and communicate effectively with the clinical team<br><br> </li> </ul><div> <strong>Qualifications<br></strong><br> </div><div>1. Must currently be licensed in the state of Texas</div><div>2. Should have at least two years clinical experience</div><div>3. Prior Geriatric home health care experience is a MUST</div><div>4. Should have Reliable Transportation & Car insurance</div><div>5. Must be able to pass background check & have valid CPR & TB test/Chest x-ray</div><div>6. Bilingual is a PLUS</div><div>8. Authorized to legally work in the United States</div><div>7.
