<div><strong>Job description</strong></div><div>Amcare Pro Home Health is a mid-size home health care company with offices in DFW & East Texas. The DFW office is currently interviewing for a <strong>Home Health RN Case Manager - PRN or Full Time </strong>to cover the <strong>Flower Mound, Grapevine, Keller, Roanoke and surrounding areas.<br></strong><br>
</div><div>The position will involve home care visits for Admits, Recerts, Sup & SN visits. Previous Home Health Care experience is a must. Familiarity with electronic charting (KINNSER/AXXESS) is a plus.<br><br>

<strong>Job Details<br></strong><br>
</div><div>The RN position will be required to perform home health SN Initial Assessments, OASIS Admissions, Re-cert & ROC visits, DC visits ensure compliance with all State regulations. The RN must have knowledge/experience with home health forms, OASIS charting, Medicare guidelines, rules & regulations.<br><br><strong>Responsibilities:<br></strong><br>
</div><ul>
<li>Conduct SOC, ROC, recertification, and discharge visits</li>
<li>Complete OASIS assessments accurately and timely</li>
<li>Develop and manage individualized Plans of Care</li>
<li>Provide skilled nursing care (wound care, medication management, disease management, IV therapy as needed)</li>
<li>Coordinate care with physicians and interdisciplinary team</li>
<li>Educate patients and caregivers on treatment plans and safety</li>
<li>Ensure compliance with Medicare and Texas home health regulations</li>
<li>Complete all documentation in the agency EMR within required timeframes<br><br>

<strong>Qualifications<br></strong><br>
</div><div>1. Must currently be licensed in the state of Texas</div><div>2. Should have at least 2 year nursing experience</div><div>3. IV infusion experience is a plus</div><div>4. Prior home health care experience is a MUST</div><div>5. Should have Reliable Transportation & Car insurance</div><div>6. Must be able to pass background check & have valid CPR</div><div>7. Experience with Oasis E and use of Wellsky/Kinnser is a plus</div><div>8. Knowledge of ICD-10 codes is a plus</div><div>9. Excellent compensation with benefits<br><br>
