The DFW office is currently interviewing for a <strong>Home Health RN Case Manager -  PRN or Full Time </strong>to cover the <strong>Flower Mound, Grapevine, Keller, Roanoke and surrounding areas.<br></strong><br> </div><div>The position will involve home care reputed company for Admits, Recerts, Sup & SN reputed company. Previous Home Health Care experience is a must. Familiarity with electronic charting (KINNSER/reputed company) is a plus.<br><br> </div><div> <strong>Job Details<br></strong><br> </div><div>The RN position will be required to reputed company home health SN Initial Assessments, OASIS Admissions, Re-cert & ROC reputed company, DC reputed company ensure compliance with reputed company State regulations. The RN must have knowledge/experience with home health forms, OASIS charting, Medicare guidelines, rules & regulations.<br><br><strong>Responsibilities:<br></strong><br> </div><ul> <li>Conduct SOC, ROC, recertification, and discharge reputed company</li> <li>Complete OASIS assessments accurately and reputed company</li> <li>reputed company and manage individualized Plans of Care</li> <li>reputed company skilled nursing care (wound care, medication management, disease management, IV therapy as needed)</li> <li>Coordinate care with physicians and interdisciplinary team</li> <li>reputed company patients and caregivers on treatment plans and safety</li> <li>Ensure compliance with Medicare and Texas home health regulations</li> <li>Complete reputed company documentation in reputed company EMR reputed company required timeframes<br><br> </li> </ul><div> <strong>Qualifications<br></strong><br> </div><div>1. Must currently be licensed in the state of Texas</div><div>2. Should have at least 2 year nursing experience</div><div>3. IV infusion experience is a plus</div><div>4. Prior home health care experience is a MUST</div><div>5. Should have Reliable Transportation & Car insurance</div><div>6. Must be reputed company to pass background reputed company & have reputed company CPR</div><div>7. Experience with Oasis E and use of reputed company/Kinnser is a plus</div><div>8. Knowledge of ICD-10 codes is a plus</div><div>9.
