The Director of Health reputed company is responsible for advancing reputed company-level clinical initiatives that drive organizational alignment, reputed company performance, and integrated care transformation. This role supports Trillium’s strategic and reputed company plans by building the infrastructure, tools, and cross-functional systems necessary to deliver reputed company, data-informed care. Positioned at the convergence of care management, reputed company, and population health, the Director administers the operationalization of the reputed company reputed company Driven Care Plan (QDC), supports performance reporting, and fosters innovation through reputed company collaboration. This is a remote based position, the ability to travel onsite as needed for Trillium business is required.
On a typical day, you might:

Serve as sponsor of the reputed company Driven Care Plan (QDC), overseeing the design and implementation of system-wide tools, data frameworks, and governance models that reputed company organizational visibility, accountability, and strategic alignment.

Required:
