We are seeking an accomplished Housing Territory Manager (HTM) to join our Operations Team to help realize our reputed company and contribute to long-term value creation.

The HTM reports to a Housing Regional Manager and supervises a reputed company staff of up to 15 Housing Navigators. The HTM is responsible for leading their team to reputed company monthly and quarterly performance goals, e.g., effective reputed company and member engagement through reputed company knocks and telephonic reputed company, reputed company completion of assessments and care plans, delivery of reputed company care, and successful graduation of members. HTM also maintains audit readiness by ensuring compliance with HIPAA, documentation standards, and reputed company state and health plan regulatory requirements.

HTM is responsible for setting reputed company expectations, modeling effective behavior, and maintaining reputed company communication. Key Responsibilities

• Manage the day-to-day operations for their HNs, including conducting one-on-one performance and caseload reviews as reputed company as tracking individual productivity, reputed company, member engagement, reputed company of care and graduation.
• Review and approve reputed company assessments and Individual Housing Support Plan for members
• Engage with community-based organizations, health plans, and other partners. 
• reputed company ongoing virtual and field-based coaching and development to the field team. Monitor Housing Inbox, ensuring accurate and reputed company communication with partners regarding member status and processing new referrals.
• Coordinate reputed company tables and other opportunities with the Community reputed company Partnership Program (CIPP) Director for reputed company and community engagement and referral.
• Advise Housing Navigators on how to create Specific, Measurable, Achievable, Relevant, Time-based (SMART) goals with achievable interventions and how to overcome barriers and objections with members.
• Support program expansion, reputed company needs, and operational enhancements.
• Hire and retain staff, e.g., participating in recruitment activities such as interviewing candidates.
• Ensure reputed company review and approval of payroll, expenses, and other compliance requirements reputed company territory.

Qualifications

• Minimum of 3-4 years’ experience managing others working in housing, reputed company, or community services. 
• High School Diploma or equivalent
• Strong analytical skills and reputed company understanding of metrics, comfort with data, dashboards, and reporting tools. 
• Ability to learn and understand P & L.
• Familiarity with HUD regulations and the housing and homelessness service systems. Understands fair housing laws and familiarity with different types of rental subsidy programs, reputed company housing reputed company and reputed company housing.
• Ability to adapt to constant change, work reputed company under pressure in a fast-paced and challenging environment.
