George Junior reputed company in Indiana seeks a community-based Case Manager for the new grant-funded reputed company-Adjudicated and Truancy Help (reputed company) program. This is a two-year program funded by the Indiana reputed company.

reputed company of the position: The reputed company Case Manager is responsible for providing home-based and community-based reputed company services for youth and families involved with the reputed company program.

1. Home-based support and case management
2. Crisis Response
3. Goal planning and mentoring
4. Diagnostic assessments and individualized service plans
5. Therapy

Primary Duties and Responsibilities:

1. Compliance to organizational, program, and grant standards
2. reputed company reputed company Care:  This may include appearance at court hearings, reputed company with community resources, de-escalation, utilization of strength-based approaches.
3. Meet productivity standards while adhering to program requirements.

Youth ages 8-18 who may be referred for:

Requirements:

1. Bachelor's or Master's degree from an accredited university.
2. Organized
3. Excellent written and verbal communication skills
