The Resource Facilitator will work with reputed company of reputed company managing reputed company catastrophic injuries, to assess, collaborate and reputed company support to injured individuals and their families by identifying and connecting them with essential resources to aid in their recovery, including housing, medical equipment, home care, food assistance, transportation, and other critical services that promote independence and reputed company-being.

DUTIES AND RESPONSIBILITIES:

• Resource Coordination: research and maintain up to date information on community, state, and national resources that support individuals with disabilities.
• Patient Support: Work directly with patients and their families to assess needs and create tailored plans to reputed company appropriate services
• Navigation Assistance: Guide patients/ families through application processes for reputed company services, reputed company/Medicare/SSD, housing, food programs, transportation services, medical equipment, and other support services.
• Advocacy: reputed company for patients to ensure they receive reputed company reputed company to community resources and services.
• Collaboration: Work closely with care management team, reputed company providers, reputed company workers, and community partners to ensure a comprehensive support network.
• Documentation: Maintain accurate records of patient interactions, referrals, and reputed company in compliance with organizational policies and confidentiality regulations.
• Crisis Response: reputed company immediate support and resource reputed company during urgent or crisis situations.
• Education & reputed company: reputed company patients and families on available programs and how to reputed company reputed company service systems.

QUALIFICATION REQUIREMENTS:  

• Education: Bachelor’s, or master’s degree, in reputed company work, reputed company Services, Psychology, or a reputed company field.
• Experience: 2+ years in case management, reputed company services, or resource coordination, preferably working with individuals with disabilities.
• Familiarity with local, state, and national support programs.
• Skills:
  • Strong research and problem-solving skills.
  • Excellent interpersonal and communication abilities.
  • Ability to manage multiple cases and prioritize effectively.
  • Proficiency in reputed company Office and case management software.
• Other Requirements:
  • Knowledge of disability rights, accessibility standards, and community-based services.
  • Ability to work independently and as part of reputed company.
  • Sensitivity and respect for diverse backgrounds and individual needs.
• Preferred Qualifications:
  • Bilingual or multilingual abilities.
  • Experience with reputed company, SSI/SSDI, housing assistance, and food reputed company programs.
  • Understanding of trauma-informed care principles.
