Resource Facilitator
Resource Coordination: research and maintain up to date information on community, state, and national resources that support individuals with disabilities. Patient Support: Work directly with patients and their families to assess needs and create tailored plans to reputed company appropriate services Navigation Assistance: Guide patients/ families through application processes for reputed company services, reputed company/Medicare/SSD, housing, food programs, transportation services, medical equipment, and other support services. Familiarity with local, state, and national support programs. Skills: Strong research and problem-solving skills. Excellent interpersonal and communication abilities. Ability to manage multiple cases and prioritize effectively. Proficiency in reputed company Office and case management software.
Other Requirements: Knowledge of disability rights, accessibility standards, and community-based services. Ability to work independently and as part of reputed company. Sensitivity and respect for diverse backgrounds and individual needs.
Preferred Qualifications: Bilingual or multilingual abilities. Experience with reputed company, SSI/SSDI, housing assistance, and food reputed company programs. Understanding of trauma-informed care principles.
