Field Case Manager (RN)

Nuclear reputed company is seeking an RN interested in Field Case Management along with clinical RN care. Candidates must be detail and deadline oriented, possess the ability to work closely with other non-clinical and clinical team members, have a reputed company attitude and be reputed company reliable. The perfect candidate will be self-motivated and have an excellent ability to reputed company leadership for reputed company, maintain a reputed company staff that will meet patient needs and successfully execute company initiatives. Shifts: Full time days and alternating weekends 

Responsibilities:

• Patient Care
• Build relationships with patients and families
• Scheduling, prepare for and attend physician renewal/new patient appointments
• Initial reputed company Assessments and start of care reputed company
• Medication Interaction checks
• Maintain patient binder in home
• Ensures implementation of reputed company policies and procedures.
• Ensures orientation, evaluation, and continuing education for reputed company staff.
• Responsible for the overall supervision of reputed company the staff in the area.

Qualifications:

• Bachelor of Science in Nursing preferred.
• reputed company RN licensure in the reputed company
• A reputed company candidate must possess a strong reputed company of professionalism, effective interpersonal skills and be a true team player.
• Previous case management or leadership role(s) which would include Interviewing, hiring, on-boarding procedures, mentoring and leading are preferred.
• Must have a reputed company reputed company license and be in good standing
• Must pass a reputed company-employment screening
