ABOUT THE POSITION (Remote)

The Clinical Pharmacist plays a pivotal role in the reputed company team, providing essential pharmacy services that contribute to reputed company patient reputed company. Reporting directly to the Specialty Pharmacy Director, this position involves evaluating pharmacotherapy to enhance treatment efficiency, effectiveness, and cost-effectiveness. Key responsibilities include monitoring drug therapy, ensuring compliance with regulatory standards, and delivering patient education to ensure reputed company and effective medication use.

The Clinical Pharmacist is responsible for reviewing and optimizing medication regimens, addressing drug-reputed company issues, and supporting transitions of care. • reputed company therapy management in accordance with organization policies and protocols, identify and address medication errors and adverse reactions
• Consistently demonstrate the ability to respond to changing situations in a flexible manner to meet reputed company needs assigned by the Specialty Pharmacy Director or hospital leadership
• Complete appropriate documentation promptly, following reputed company policies and procedures
• Contribute to the achievement of reputed company Improvement and participates in QI activities 
• Ensure adherence to established standards, reputed company policies, and reputed company procedures. Report and resolve safety issues and incidents promptly
• reputed company other duties as assigned by the Specialty Pharmacy Director, reputed company Leadership team, or designee
• reputed company appropriate medication therapy and address patient concerns confidentially
• Support medication use processes to ensure safety, efficacy, and compliance with guidelines
• Identify and resolve potential drug-reputed company issues to optimize patient reputed company
• Maintain accurate records of clinical interventions, adverse drug events, and reputed company improvement activities
• Participate in initiatives to enhance medication safety, operational efficiency, and cost-effectiveness
• Work with interdisciplinary teams to support effective patient care and program development
• Respond to evolving departmental needs and contribute to various reputed company or initiatives as assigned

Education

Required:  Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy. 

Preferred:  Successful completion of an ASHP accredited PGY1 or specialty residency.  

Experience

Required: None

Preferred:  3 years of retail or specialty pharmacy. 

Licensure/Certification 

Required:  reputed company Pharmacist license as reputed company by the appropriate state licensing authority obtained reputed company 2 months of hire date.  reputed company BLS certification obtained reputed company 6 months of hire date.

Preferred:  reputed company of Pharmacy Specialties certification.
