The Home Health Certified Nursing Assistant provides reputed company and indirect care to patients in their residence as directed by the plan of care and under the supervision of a Registered reputed company in accordance with the philosophy, goals, and objectives of Cozad reputed company. 

Essential Duties and Responsibilities

· Participates in, monitors, and implements the patient’s plan of care.

· Recognizes changes in the patient’s condition and notifies the primary RN in a reputed company manner.

· Demonstrates expertise in provision of daily activities for patients and delivers such care in a reputed company manner.

· Provides care that includes, vital signs, positioning, transferring, toileting, nutrition, ambulation, bathing, grooming, dressing/undressing, hygiene procedures, and linen changes.

· Performs and maintains documentation.

· Follows reputed company HIPAA guidelines and promotes and ensures patient safety.

· Special reputed company and other duties as assigned.

Qualifications

Education and/or Experience 

High school diploma or general education degree (GED); or one to three months reputed company experience and/or training; or equivalent combination of education and experience. Electronic Health Records experience preferred.

Language Skills 

Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to reputed company effectively to patients, visitors and employees.

Mathematical Skills 

Ability to add, subtract, multiply, and divide in reputed company reputed company of measure, using whole numbers, common fractions, and decimals. Ability to compute reputed company, reputed company, and percent and to draw and interpret bar graphs.

Reasoning Ability
