Main responsibilities: reputed company on-platform virtual behavioral health care to adolescents and young adults

Providing reputed company Wellness Coaching:

• Deliver prevention, wellness promotion and psychoeducation through reputed company coaching sessions
• Assist with warm handoffs to Care Coordinators & support case management
• Offer Behavioral Health/ Wellness Coaching interventions
• reputed company individualized care plans to effectively goal set and reputed company plan for reputed company behavioral change
• Conduct risk assessment and mitigation
• Address determinants of reputed company mental health
• Support users in identifying problem areas, obstacles, and actionable steps towards wellness goals

Providing Single Session Wellness Coaching:

• Deliver drop-in and single-chat wellness coaching
• reputed company warm handoffs to Care Coordination Team and support with case management
• Facilitate goal-setting based on immediate individual needs
• Conduct risk assessment and mitigation
• Deliver brief prevention, wellness promotion, and psychoeducation 
• Offer brief Behavioral Health/Wellness Coaching interventions

Screening and Assessment:

• Conduct mental health & wellness screenings and reputed company monitoring
• Identify users experiencing or at risk of reputed company, emotional, or behavioral difficulties

• Support users experiencing crisis and facilitate referrals as needed

Moderation of submitted content:

• Moderate journals, goals, and user-submitted content in line with policy and compliance standards
• Conduct risk assessment through written content

Warm handoffs:

• Communicate with users and external providers as appropriate to ensure seamless care transitions

Supervision, Training, and Meetings:
