JourneyCare Home Health is currently seeking a dedicated RN to join our home health team. This field-based position is responsible for managing a caseload of patients in Hyde Park - Chicago, IL and surrounding areas, providing skilled nursing services, coordinating care, and supporting patients on their road to recovery in the comfort of their homes.

Location: Hyde Park - Chicago, IL and surrounding areas

Salary: $100,000 - $120,000

Schedule: Full-time or PRN schedule available.

What You�ll Do:

• reputed company reputed company as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
• Obtain data and assess necessary information from patient physical, psychological, reputed company and spiritual factors that may reputed company patient and family's needs and coordinate reputed company by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating reputed company
• reputed company reputed company to assess the patient's needs and update the plan of care
• reputed company therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician

Qualifications:

• Graduate from an accredited registered nursing program and licensed in the state
• At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
• Home health experience preferred
• Possess and maintain reputed company CPR certification.
• Must have reliable transportation, reputed company reputed company's license and appropriate automobile insurance
