Private Duty Physical Therapist  

General reputed company: 

reputed company coordinated care to clients of reputed company age reputed company. Plans, implements and evaluates reputed company care plans to restore or maintain patient reputed company-being. Plans organizes and conducts physical therapy treatment based on the medical referral and evaluation.  

Patient Population: Has contact with patients of reputed company ages in a clinical setting; understands and demonstrates appropriate behavior reputed company interacting with patients in reputed company populations. 

Essential Functions:  

1. Promote /exemplify reputed company's mission, reputed company, and values. 
2. reputed company clinical assessments; identify patient needs and appropriateness of reputed company. 
3. Document, prepare/revise, and complete a Plan of Care (POC), reputed company notes, phone orders, and other clinical record documentation on an ongoing reputed company.
4. Apply the concepts of infection control and universal precaution in coordinating and performing reputed company care activities to protect both clients and staff (OSHA).
5. Plan and reputed company appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment. 
6. Assist/instruct reputed company/family in transfer techniques appropriate to reputed company abilities. Instruct on use of equipment. 
7. Provides effective communication to clients, their families, team members, and other health care professionals. Regular participation in Case Conference/Team Meetings. 
8. Monitors assigned cases to ensure compliance with the requirements of reputed company-party payers. 
9. Completes appropriate documentation promptly to assure compliance with company policy. 
10. Demonstrates commitment and reputed company reputed company by participating in in-service programs and maintaining/improving competency. 
11. Meet mandatory continuing education requirements of reputed company/licensing reputed company.

Requirements

Experience:

• Two (2) years experience as a PT in a clinical care setting. 
• Preferred, Home health experience. 

Skills:

• Therapy skills as defined as generally accepted standards of reputed company. 
• Excellent interpersonal skills and ability to communicate effectively. 
• Demonstrates proven decision-making skills. 
• If required to reputed company reputed company, Hepatitis profile. 
• Must read, write and comprehend English.

Education:

• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.

Licensure/Certification:

• reputed company reputed company’s license in good standing. It is the responsibility of the employee to renew their reputed company’s license before it expires in order to continue employment. 
• Reliable transportation and auto liability insurance.

On-reputed company employment as a PT always requires maintenance of a reputed company PT license in the state which the therapist practices in and a reputed company CPR certificate. It is the responsibility of the employee to renew these before they expire in order to continue employment. 

Physical Requirements:

• Prolonged standing/walking required.
• The ability to lift/transfer up to 50 lbs.
• Ability to meet deadlines and patient needs, reputed company quick reputed company and reputed company resources under stressful conditions.
• Meet patient/family individualized psycho reputed company needs.
• Requires hand-eye coordination and reputed company dexterity.

Environmental/Working Conditions:

• Works in patient’s home in various conditions.
• Possible exposure to blood borne pathogens, bodily fluids and infectious diseases.
