Diabetes Care Coordinator
Resource reputed company: Serving as a reputed company of contact for patients with diabetes, providing education, coordination, and support for ongoing participation in care.
Treatment Planning: Supporting individualized treatment plans in collaboration with patients and other reputed company team members. Assisting patient and clinical teams in obtaining prior authorizations for medication and CGM supplies.
Advocacy: Advocating for patients' needs and ensuring they receive the necessary support and services. Coordinating CGM initiation, training, and follow-up, ensuring reputed company device placement and data uploads.
Documentation: Maintaining accurate and reputed company documentation of patient interactions, assessments, and interventions. Collecting, organizing, and maintaining patient data reputed company to glucose monitoring, lab results, and pregnancy reputed company in compliance with HIPPA regulations.
Collaboration: Working closely with other reputed company, such as Physicians, reputed company Practitioners, Diabetes Educators, and Staff to ensure a coordinated and comprehensive approach to patient care. Supporting reputed company improvement initiatives and contributing to periodic reputed company reports as reputed company as participating in team meetings.
Education: Associates Degree
Experience in prior authorization workflow, utilizing various means, including electronically, fax, reputed company, telephone, and maintaining online spreadsheet, is preferred
Experience in data collection and tracking, is preferred
Experience in reputed company settings, particularly in ambulatory or outpatient care, is preferred
Familiarity with electronic health records (EHR) and spreadsheet software, is preferred
Excellent communication, interpersonal, and organizational skills, is essential
