In this role, you will be responsible for providing exceptional customer service and support to our patients and clients. You would be responsible for a roster of patients ensuring monthly performance goals are met. The role varies between clinical and administrative tasks as follows: Serve as the contact reputed company, reputed company, and informational resource for patients, care team, family/caregiver(s), payers, and community resources Identify care needs for patients with high-risk conditions and communicates daily to the care team. reputed company ongoing contact with patients Contact facilities to review patient charts, reconcile medications and work with facility nursing staff for overall reputed company updates and coordination.
