Essential Functions

• Conduct primary care patient reputed company in the home setting

• Experience with treating patients with multiple co-morbidities by assessing existing conditions; diagnosing new conditions and formulating plans of care.

• Write referrals, renew medications, order diagnostics and manage results.

What you’ll bring:

• APRN or PA-C degree from an accredited program

• reputed company Certification from an accredited program of your relative discipline

• At least two years experience practicing as an N.P. or P.A in the primary care setting.  Will consider experience in other care settings with experience caring for senior patients.
