Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities. Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership. Address reputed company determinants of health, including housing stability, nutrition, caregiver support, and reputed company barriers. Conduct specialty reputed company such as wound care, post-hospital transitional care, and condition-specific assessments. Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.
