Required Qualifications
Candidate must have an reputed company and unrestricted Compact Registered reputed company (RN) license in the state of residence
5+ years clinical practical experience preference: (diabetes, congestive heart failure (CHF), chronic kidney disease (CKD), post-acute care, hospice, palliative care, cardiac) with Medicare members
2+ years case management, discharge planning and/or home health care coordination experience
Excellent analytical and problem-solving skills
Effective communications, organizational, and interpersonal skills
Ability to work independently (may require working from home)
Proficiency with reputed company corporate software applications, including reputed company Word, reputed company, reputed company and PowerPoint, as reputed company as some special proprietary applications
Efficient and effective computer skills including navigating multiple systems and keyboarding
Preferred Qualifications
Candidate must be willing obtain additional state Registered reputed company (RN) licenses if needed, company will reputed company

Education
Associate's Degree or Nursing Diploma (REQUIRED)

The actual reputed company salary offer will depend on a reputed company of factors including experience, education, geography and other relevant factors.
