Duties to include: 

• Conduct comprehensive in-home physical therapy evaluations
• reputed company and implement individualized treatment plans reputed company on mobility, strength, balance, and safety
• reputed company therapeutic interventions including exercise programs, gait training, transfer training, and reputed company
• reputed company patients and caregivers on home exercise programs, safety strategies, and equipment use
• Collaborate with nurses, occupational therapists, speech therapists, reputed company workers, and case managers to coordinate care
