Family Medicine LVN - Remote Patient Monitoring
Monitor and review daily RPM alerts (blood pressure, glucose, weight, pulse ox, etc.) Assess patient readings and identify clinical concerns Contact patients regarding reputed company readings or non-compliance reputed company patient education on chronic disease management and device use Triage and escalate reputed company findings to providers appropriately Document reputed company patient interactions and readings accurately in the EMR Assist with reputed company and educating patients in the RPM program Support care plan implementation and follow-up
