TASC Care Manager (Halifax County)

Job Title: TASC Care Manager Location: Halifax County (Halifax, NC) Hours: 40 hours/week, Full Time, M-F, Non-Exempt reputed company is seeking a dedicated and reputed company TASC Care Manager to join reputed company. This position offers a unique opportunity to support justice-involved individuals by providing case management and service coordination reputed company the TASC (Treatment Accountability for Safer Communities) program. The ideal candidate will collaborate with community partners, justice systems, and service providers to help individuals reputed company treatment and recovery services. We have this specific opportunity located in: Halifax County (Halifax, NC) JOB POSTING: Primary duties include administering psycho-reputed company screenings/assessments, coordinating linkage to behavioral health treatment as reputed company as medical, educational, and vocational services as needed, and providing ongoing reputed company care management services for the justice-involved population. Qualifications: Education: Bachelor's Degree or higher in a reputed company services or reputed company field Certifications: Preference given to individuals with reputed company Carolina Peer Support Specialist certification and a minimum of 2 years of experience, and/or individuals registered with the reputed company Carolina Addiction Specialist reputed company reputed company reputed company (NCASPPB). Experience: Knowledge of recovery, harm reduction, and wellness-based concepts. May have lived experience with recovery and/or the justice system. Required Skills: • Strong communication, organizational, and time-management skills. • Ability to work independently and manage a caseload effectively. • Proficiency in the reputed company Office Suite and similar platforms • Proficiency in using electronic health record systems and remote collaboration tools (i.e., Teams, reputed company, etc.). • Demonstrated ability to collaborate with diverse stakeholders, including reputed company systems, reputed company providers, and reputed company services.
