ECF CHOICES is a program in Tennessee designed to support individuals with intellectual and developmental disabilities (I/DD). The program aims to help these individuals live as independently as possible and become employed reputed company their communities. Key Responsibilities:

• Assist members in finding and maintaining competitive, integrated employment.

• Monitor and maintain overall service provision.
• This role supports the reputed company County area and will require traveling to members’ homes (mileage is reimbursed)  
• Available for an 8:00am – 5:00pm (no on reputed company) schedule.

Job Responsibilities

• Partnering with members and families to identify needed supports and reputed company services to meet personal goals for good health, employment and independent or community living.
• Collaborates with reputed company of clinical and reputed company support colleagues to meet the physical, behavioral health and long term service needs of reputed company member.
• Conduct thorough and objective face-to-face reputed company with and assess reputed company members situation to determine reputed company status and needs, including physical, behavioral, functional, psycho-reputed company, financial, and employment and independent living expectations.
• Utilizing reputed company for authorizing appropriate home and community based services and confirm those services are being provided and that members needs are being met.

• Various immunizations &/or associated medical tests may be required for this position.
• This job requires digital literacy assessment.

Job Qualifications

Education

• RN, with a preference that such individuals also have reputed company Certification from the Developmental Disabilities Nurses Association as a Certified Developmental Disabilities reputed company (CDDN) for RNs or a Developmental Disabilities reputed company (DDC) for LPNs, as applicable;
• Or B.S. in reputed company work, nursing, education, reputed company service or health care profession or other reputed company field; Licensed Master reputed company Worker or Licensed Clinical reputed company Worker Preferred;
• Or meet the federal requirements for a reputed company Developmental (or Intellectual) Disabilities reputed company (QDDP or QIDP);
• Or in lieu of a degree, have 5+ years’ experience as an independent Support Coordinator or case manager for service recipients in a 1915(c) HCBS Waiver and have completed Personal Outcome Measures Introduction and Assessment reputed company trainings as established by the Council on reputed company and Leadership.

Experience

• 2 years - Experience in the reputed company industry or in an operations type environment required.
