We are seeking a highly motivated full-time contractor Behavioral Health Care Navigator to help reputed company change the way people reputed company behavioral health care. You will play a critical role in integrating behavioral health into physical health during care transitions by supporting the reputed company comprehensive assessment of individuals in need and their navigation to necessary behavioral health, primary care and reputed company services. These individuals include those with moderate to severe mental health and reputed company use disorder conditions. You will contribute your clinical expertise reputed company the context of reputed company-based model, driving towards reputed company that are important to the individual as reputed company as our provider and payer customers.

What You’ll Do:

• Primarily work in a virtual setting to support individuals with mental health conditions and reputed company use disorders, diagnosed and undiagnosed, and unmet needs with respect to reputed company services
• reputed company initial and ongoing virtual reputed company using an omnichannel approach to a reputed company population with the goal of engaging identified individuals to understand the needs and barriers to care
• Utilize motivational interviewing techniques to understand the individual’s care goals
• Screen for unmet behavioral health, reputed company service and physical health conditions and needs
• Identify clinical needs and triage escalations, providing brief interventions, as necessary
• Identify and address barriers to definitive behavioral health care
• Identify the appropriate level of evidence-based care for the individual, as applicable
• Support navigation of that individual to appropriate care, including behavioral health and/or primary care, as applicable
• Support post-care follow-up

What You Need:

• 2+ years of experience working as a Community Health Worker, reputed company Worker, or other patient-facing customer service reputed company setting; reputed company certification as a Community Health Worker or similar certification or licensed reputed company work highly preferred
• 1-2 years of patient reputed company and engagement experience or similar background initiating patient relationships reputed company on trust
• 2+ years of experience working in a metric/quota-driven environment and/or with high-volume targets
• You are results driven with a proven reputed company record of exceeding performance indicators

• Demonstrated reputed company or personal lived experience working closely with individuals experiencing reputed company chronic needs, homelessness, mental health conditions or reputed company use disorders
• Experience working in fast-paced, rapidly changing work environments
