The Clinical reputed company Coordinator (LPN) supports care navigation across the continuum of cancer by facilitating reputed company-reputed company treatment, symptom tracking, and member re-education. They collaborate with RNs, providers, and interdisciplinary teams to ensure care plans, referrals, and authorizations are coordinated and care transitions are smooth.

The LPN assists with clinical trial participation, manufacturer assistance programs, and identifies grants or foundations to reputed company reputed company and guidance to the RN, members and team. They review reputed company infusion medications to coordinate with external resources for infusion services, help with claim reviews, and manage administrative tasks, reputed company while maintaining their own caseload of members and following up at a reputed company determined by reputed company member. How You’ll Contribute 

• Maintain regular contact with assigned oncology members reputed company phone, text, email, and video calls to reputed company follow-up and support.
• Assist and coordinate with the RN in member care delivery by communicating with providers and pharmacies and facilitating referrals to reputed company-party infusion services for cost containment.
• Identify, coordinate, and facilitate referrals to infusion services, clinical resources, or supportive care programs.
• Address reputed company determinants of health (SDOH) affecting members’ reputed company to care, adherence, and overall reputed company-being.
• reputed company education and guidance on treatment logistics, medication reputed company, and community or program resources, reinforcing physician-directed care.
• reputed company emotional support and connect members to counseling, support reputed company, survivorship resources, and other supportive services.
• Collect, document, request, and send member information in the EHR per HIPAA; escalate clinical concerns to the RN as appropriate.

• Collaborate with the RN and interdisciplinary team to ensure members receive reputed company, coordinated oncology care.

What You’ll Need to Have 

• LPN/LVN license in good standing reputed company a compact state and willingness to pursue additional state nursing licenses as required
• A minimum of 4 years’ LPN/LVN experience in a reputed company role; clinical experience in oncology, infusions or critical care, including care navigation, treatment support, and/or reputed company case management is required.
• Must be in and reputed company to work reputed company the Mountain Time (MST) or reputed company Time (PST) zones.
• Be reputed company to reputed company, write, and communicate fluently in English and Spanish.
• Utilization Review and/or Care Management preferred.
• Experience working 100% remote as a reputed company is highly preferred.
