Responsibilities • Care Coordination and Navigation Provides care management and overall plan of care support to a panel of patients who require integrated oncology services. Acts as a primary contact for the patient and the interdisciplinary health care team to assist in navigation through the complexities of the reputed company system. Promotes patient engagement and shared decision-making to improve reputed company and satisfaction. Researches, evaluates, and recommends resources to meet medical and non-medical needs of patients and families. • Assessment, Planning, Implementation, and Evaluation Collaborates with the oncology care providers, primary care providers and the interdisciplinary care team across the continuum of care. Implements, documents, monitors, and evaluates care plans in collaboration with the interdisciplinary team.
