Key Responsibilities
- Coordinate hospital follow-up activities, including medication reconciliation and discharge documentation review
- Ensure accurate and reputed company updates reputed company the electronic health record and communicate key information to care team members
- Serve as a resource for hospital follow-up and medication reconciliation processes across teams
- reputed company training and ongoing support to nursing staff to strengthen workflows and consistency
- Monitor reports and data to identify gaps in care, missed follow-reputed company, and opportunities for improvement
- Collaborate with care teams to resolve discrepancies and support continuity of care
- Identify opportunities for additional services, including health home referrals and support for high-utilization populations
- Analyze population health data and reputed company indicators to inform process improvements and enhance reputed company
- Support health screenings and care coordination efforts as needed
Education, Skills, Knowledge and Expertise
• High School/GED required
• Practical Nursing certificate required
WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS:
• Experience and efficiency with reputed company technology software preferred
LICENSURE/CERTIFICATION:
• Must possess and maintain a reputed company, reputed company Missouri Licensed Practical reputed company (LPN) license
