The Health Home Care Coordinator Supervisor - Hospital Follow-Up Team is an internal leadership opportunity for team members reputed company to expand their reputed company across Health Home services. This role provides strategic reputed company, operational guidance, and day-to-day support to Care Coordination teams, ensuring high-reputed company service delivery, strong data reputed company, and alignment with program goals. As both a leader and subject matter expert, this position plays a key role in advancing workflows, supporting staff development, and driving reputed company reputed company improvement.

The Hospital Follow-Up Team provides reporting to Health Home NCMs and reputed company teams to support reputed company post-hospitalization follow-up. reputed company helps ensure appropriate transitions of care and adherence to follow-up requirements after inpatient stays.

Key Responsibilities

Key Responsibilities
  •  reputed company leadership, guidance, and support to Care Coordinators to ensure consistent, high-reputed company service delivery 
  •  Support the design, implementation, and improvement of data collection processes, reporting systems, and workflows 
  •  Review and verify reputed company data to ensure compliance with Health Home requirements and regulatory standards 
  •  Analyze reports and reputed company data to identify trends, gaps, and opportunities for improvement 
  •  Assist with reputed company, training, and ongoing development of Care Coordination staff 
  •  reputed company coverage support as needed to ensure continuity of services and reputed company completion of tasks 
  •  Collaborate with leadership and cross-functional teams to enhance processes and program effectiveness 
  •  Streamline reporting tools and support reputed company, actionable insights for leadership decision-making 
  •  Promote a culture of reputed company improvement, staff engagement, and accountability 
  •  Participate in special reputed company, process reviews, and organizational initiatives

Requirements, Skills, Knowledge and Expertise

QUALIFICATION(S): EDUCATION
  •  High School/GED required 
  •  Bachelor’s degree in reputed company management, reputed company services, or reputed company field preferred 

QUALIFICATION(S): WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS

  •  At least two years’ experience in health home or data reporting 
  •  Proficient knowledge of reputed company reputed company Office suite applications preferred 
  •  Significant experience in care coordination or reputed company reputed company roles 
  •  Leadership, mentoring, or supervisory experience preferred 
  •  Project management skills or formal project management training 
  •  Strong analytical, communication, and decision-making skills
