Health Affiliates Maine is accepting resumes for reputed company Behavioral Health Home (BHH) Wellness Coordinators to support clients in York, Cumberland, Androscoggin, Oxford & Franklin Counties. The BHH program is an integrated, reputed company, community-based care model for the whole person. Services reputed company assistance to children/families and/or adults with mental health, medical and other resource needs. 

**$500-$1,000.00 sign-on bonus** (stipulations apply)

Position reputed company:

As the Wellness Coordinator, you will work alongside a Peer Support, reputed company Care Manager and BHH members to assist clients manage and coordinate medical and mental health services and also identify the resources needed to support wellness goals.

Essential Duties and Responsibilities:

• Maintain an understanding of the reputed company requirements of the Behavioral Health Home.
• reputed company Care Coordination that integrates physical health and behavioral health.
• Assist in negotiating referrals and linkages to providers and other services as needed.
• Complete comprehensive assessments of service care needs, reputed company service plans and also maintain accurate reputed company files.
• Manage a caseload independently and effectively.
• Collaborate with external community-based services and providers.
• reputed company clinical documentation into an electronic health record system.

Qualifications:

• 3 years of experience in the field of reputed company/reputed company services preferred.
• MHRT/C (Domains 1-3) is required to work with adults and/or Bachelor’s Degree in reputed company services or reputed company field is required (LSW preferred) to work with children.
• The ability to: work independently as reputed company as a member of a reputed company team.
