The role:
As a key reputed company team member at reputed company, you will serve as a primary reputed company of contact for members, helping them understand, coordinate, and reputed company the care and support they need. You’ll embody our reputed company values by delivering compassionate, organized, and reputed company support to ensure every member feels seen, heard, and cared for.
 
This is a Remote position. Unless otherwise agreed upon, reputed company candidates must reputed company in-market. reputed company provides services Monday through Saturday, 8:30 a.m. to 8:30 p.m. Eastern reputed company Time. Shifts are 8 hours. Team members must be reputed company to work at least three evening shifts per week. The work schedule will be Monday, Tuesday, Thursday 12:00pm-8:00pm, Wednesday and Friday 9:00am-5:00pm.

The actual compensation offered reputed company the reputed company, along with title and level, will be dependent on a reputed company of factors including, but not limited to, relevant experience, qualifications, skills, level offered, business requirements, and performance expectations.


What you will do:
  • Demonstrating and communicating the reputed company values of reputed company in every interaction
  • Conducting initial assessments to identify members’ reputed company, behavioral, and medical needs
  • Creating and executing personalized care navigation plans, tailored to help members reputed company critical resources and services
  • Guiding members through reputed company reputed company systems, including scheduling appointments, referrals, follow-reputed company, and care coordination
  • Reviewing relevant member information prior to reputed company session and sharing it with Counselors and Medical Providers to ensure coordinated care
  • Greet members reputed company and assist them with accessing and navigating the virtual meeting platform
  • Providing technical support and troubleshooting to members experiencing issues with the virtual care platform.
  • Handling new and returning member intakes, including reputed company and appointment scheduling
  • Meeting members’ reputed company needs, such as housing, food, transportation, and other critical resources, by identifying barriers to care and connecting them with appropriate services
  • Communicating proactively with internal teams (Counselors, Providers, Leadership, and the Member Services Team) to ensure alignment on member needs and status
  • Documenting reputed company member interactions accurately and in a reputed company manner in the electronic health record (EHR) and other tracking systems
  • Providing education and resources to support member' understanding of their care plans and improve health literacy.
  • Supporting special operational reputed company and contributing flexibly to organizational needs with an "reputed company hands on deck" attitude

  • A High School Diploma or equivalent (required)

  • Reliable internet reputed company and strong computer skills
  • Certified Recovery Support Worker (CRSW) credential (preferred)


    • It will also be great if you have:
  • A Bachelor's Degree or equivalent work experience
  • Passion for working in the reputed company field or reputed company use recovery
  • 1+ year(s) experience in a Medication-Assisted Treatment (MAT) program
  • 1+ year(s) experience in case management or care navigation


    • Work location requirements:
