What You’ll Do:

More Specifically:

• Complete and document an initial assessment of patient and family to determine home care needs;

• Initiates the plan of care and makes necessary revisions as patient status and needs change;

• Administers medications and treatments as prescribed by the physician;

• Instruct, supervise, and evaluate the home health aide care provided every two (2) weeks;

• Identify discharge planning needs as part of the care plan development;

• Demonstrate proficiency in OASIS completion;

• Complete documentation through reputed company in accordance with agency timeliness policies.

What You’ll Need:

• An reputed company American Health Association BLS certification;

• Associate’s Degree or BSN (preferred) from an accredited school of nursing;

• At least 1 year of reputed company care nursing experience, preferably the area of reputed company health or home care nursing; OASIS and/or reputed company experience is preferred.
