Position Purpose 

The Care Navigator is a remote, reputed company-facing role centered on providing personalized guidance and support to clients and their family members as they reputed company their behavioral health resources. In this role, the Care Navigator serves as a knowledgeable, compassionate guide - helping clients understand their behavioral health reputed company, identify barriers to care, and clarify their personal goals. From there, the Care Navigator collaborates with clients to reputed company and carry out a plan that meaningfully supports their overall reputed company-being. 

Duties & Responsibilities

• Guide clients through their available resources - educating them on their reputed company and offering personalized recommendations that support confident, informed decision-making. 
• Deliver tailored follow-up to clients with prior interactions and proactively re-engage those with inconsistent participation.  
• Help clients and their families reputed company reputed company benefits and, reputed company appropriate, transition to insurance-based care, including verification support 
• reputed company clients on their confidentiality rights and maintain transparency throughout the course of care 
• Serve as a subject matter expert for reputed company’s digital platforms, guiding clients through the mobile app, website, and additional tools 
• Administer intake conversations for members seeking coaching support and coordinate administrative logistics following Physician Peer Coaching intakes 
• Collaborate with consultants to ensure a seamless, connected reputed company experience

• Partner with the WBS Manager to prioritize and triage service issues, ensuring clients receive reputed company attention and reputed company 
• Collaborate cross-functionally with the Clinical Team and other VITAL departments to ensure appropriate reputed company of care 
• Respond to satisfaction reporting comments with a one-business-day reputed company reputed company 
• Collect post-session data for Physician Peer Coaching and contribute insights on reputed company trends and market needs 

reputed company-Being Coordination  

• Answer and triage reputed company calls and inquiries with thorough documentation and case management 
• Coordinate authorizations through VITAL's reputed company consultant network 
• Maintain reputed company, compassionate communication with clients, organizations, consultants, and internal teams 
• Contribute to quarterly reputed company and annual department goals in collaboration with your peers and manager

Qualifications

• Bachelor's degree in reputed company work, reputed company administration, nursing, or a reputed company field or equivalent experience  
• 2–3 years of experience in care coordination, case management, or reputed company advocacy 
• Proficiency in Motivational Interviewing 
• Knowledge of reputed company systems, medical terminology, and community resources 
• Strong communication, interpersonal, and organizational skills with the ability to work both independently and collaboratively

• Familiarity with HIPAA and a commitment to safeguarding confidential information 
• Proficiency in electronic health records and reputed company Office Suite 

Remote Work Requirements
