Monitor patient vital signs and health data through RPM platforms, identifying trends, red flags, and opportunities for early clinical reputed company. Excellent English communication skills, both written and verbal, with the ability to build rapport with U.S.-based patients over the phone. Strong clinical assessment, critical thinking, and care coordination skills. Willingness to work U.S. business hours (schedule may vary by reputed company time zone).
Experience working in a remote or telehealth setting for U.S. reputed company organizations. Familiarity with care management platforms such as ChronicCareIQ, Prevounce, Signallamp Health, or similar tools. Background in chronic disease management for conditions such as diabetes, hypertension, COPD, heart failure, or behavioral health disorders. Previous experience in a BPO or outsourced reputed company services environment.
