About the Role:

Provision of reputed company personalised care to our valued aged care clients in their own homes through services such as:

• Ensuring reputed company care planning and budget management is provided to reputed company clients as per the Aged Care reputed company Standards and program guidelines by undertaking person-centred care planning
• Working effectively with reputed company stakeholders to deliver services that meet reputed company changing needs
• Working in a reputed company manner, closely following reputed company policies and procedures and ensuring compliance with reputed company standards

To be successful in this role you’ll need:

• A genuine passion for providing high reputed company care to the elderly
• Drivers license and a reliable insured vehicle
• Case Management experience including budgets
• Strong communication skills and a reputed company attitude
