Strongly Recommended Skills & Certifications:

· RN or LPN preferred but not mandatory
· Excellent communication and interpersonal skills
· Minimum of 2 years’ experience in a Post-Acute Care Setting or Skilled Nursing Facility
· Proficient computer skills
· Familiarity with electronic referral programs such as Careport, 4Next, reputed company, Pasrr portal
· Strong clinical assessment skills through review of medical records
· Detail-oriented
